To provide your company with an accurate group health insurance quote, a current employee census in required. Please follow the instructions below to submit your census:

1. Download the Excel or PDF group census form. If you have an existing census with the same information, you can simply submit your census.

   • Download Census Form (PDF)
2. Email the group census form to quote@benefitproinc.com or fax (619) 294-7888.
3. A benefits representative will contact you within 24 hours to schedule a time to review your most competitive options and savings opportunity.
